If your student has any medication that needs to be brought into the health office, please include all paperwork, signed by your student's physician.
Please note: In a previous email, the student medication forms were linked incorrectly. All necessary medical forms, to be signed by your student's physician, are now available here for download.
As a reminder, families may deliver immunization and/or exemption forms as well as student medications to Health Technician, Mrs. Luquette, at the following times:
